The National Uniform Claim Committee in 2006 released a revised version of the 1500 Healthcare Insurance Claim Form to accommodate the reporting of the national provider identifier (NPI). All specialists must begin using the revised version of the 1500 claim form by April 1, 2007. The current issue of the American Medical Association’s CPT Assistant Bulletin is devoted to information on the revised 1500 form and includes steps for practitioners to successfully implement the claim form in their office.
