In February 2011, the AASM introduced an accreditation program for Out of Center Sleep Testing (OSCT) for Adult Patients. Facilities currently accredited by the AASM as a sleep disorders center are eligible to be awarded accreditation as an OCST provider by completing an Out of Center Sleep Testing Accreditation Participation Attestation Form by December 31, 2011. All AASM accredited centers must complete this form; if your center does not currently perform OCST, completion of this form is still required.
The AASM requests that centers complete their forms based on your facility’s current status as an OCST provider:
If your facility DOES currently perform OCST and wishes to become accredited: Send your completed Out of Center Sleep Testing Accreditation Participation Attestation Form to the AASM along with your $250.00 application fee. Note: After December 31, 2011, a full application for OCST accreditation must be completed to earn accreditation.
If your facility DOES NOT currently perform OCST: Send your completed Out of Center Sleep Testing Accreditation Participation Attestation Form to the AASM (no fee required). Note: If your facility does not currently perform OCST but is planning to add this service in the future, please submit your form indicating that you do not currently perform OCST. If your facility begins to perform OCST before the December 31, 2011 deadline, you may resubmit the form once you have begun providing these services. If you will not be performing OCST before December 31, 2011, you must return the attestation statement indicating that you do not perform OCST and complete a full OCST accreditation application after you have begun offering these services.
All centers must send their completed attestation form to the AASM by December 31, 2011. Forms may be sent to the AASM by fax or mail (please do not submit forms by email) to:
