The Journal of Clinical Sleep Medicine, the official publication of the American
Academy of Sleep Medicine, provides the latest original findings with the
regard to the clinical practice of sleep medicine. The Journal of Clinical
Sleep Medicine is a Bi-Monthly publication. Subscription forms received after
November 30, 2007 cannot be guaranteed delivery of all issues for the 2008
volume year. Subscribers should expect to receive the first issue within 90
days of receipt of payment. Claims for missing issues must be received within
60 days of publication date.
Changes of address should be submitted four weeks in advance of the change to
ensure uninterrupted service. Send your current mailing label (showing the old
address), along with your new address and the effective date of change.

Subscription agencies providing over 50 subscriptions to the JCSM should contact
the AASM National Office at (708) 492-0930 for agency subscription rates.
Subscriptions are prorated to the amounts below from June 16, 2008 to December 31, 2008. If you are subscribing outside of these dates you must pay the full year subscription amount for 2009. Beginning October 1, 2008, subscriptions for the 2009 volume year will be accepted.
