Personalized Brochures
Put your patients at ease by providing them with the information that they need to know about sleep and sleep disorders through these convenient, easy-to-read educational brochures. AASM patient brochures cover topics including definitions, symptoms, treatment options, tips, and answers to frequently asked questions.

Personalize your brochure order to include your contact information. Personalization allows you to:

  • Keep your contact information readily available to your patients;
  • Gain exposure through patient referrals;
  • Pass your information on to the public through direct mail, health seminars, or other public speaking engagements;
  • Create a link between yourself and the valuable information that is found within the patient brochure; and
  • Encourage follow-up appointments by making your contact information accessible to your patients.

ACCREDITED MEMBER CENTERS/LABORATORIES...
  • Must be a member of the AASM to purchase personalized brochures.
  • Are allowed to list the name of their sleep center/lab.
  • Can use the name of their sleep center in conjunction with the AASM Member Logo* as shown under the design template.
  • If including physicians' names, all listed names must be AASM individual members.

INDIVIDUAL MEMBERS...
  • May print member's name and address only. (Name of sleep lab or center may not be included).
  • May not use the AASM logo.

COST/SHIPPING...
  • All brochures are sold in lots of 50 at $15.00 per lot, plus shipping. There is a U.S. ground shipping rate chart located on the Order Form. (For orders over 3,000 brochures and outside the continental United States shipping will be quoted separately.)
  • There is a minimum order of 500 brochures per order.

PAYMENT...
  • The AASM accepts checks, credit cards (Master Card, American Express, Visa accepted), and purchase orders ($500 minimum). Checks must be in U.S. dollars, drawn on a U.S. Bank and made payable to the AASM. Purchase orders require a $500 minimum order, and a hardcopy of the purchase order must be submitted with your order.
  • The order form must be filled out completely and submitted with all orders.

TEMPLATE SUBMISSION AND REVISIONS...
  • Note in the template box on the Order form if the template is on file at the AASM National Office, new or revised.
  • For only $65.00, the AASM will create a template for you. Simply choose from one of three design templates and fill out the information you would like imprinted on your personalized brochure.
  • Camera-ready personalized templates can be attached in an e-mail using one of the following file formats: PDF, JPEG, or TIF. ***Files MUST be 300 dpi or higher. Text files should be submitted as Word documents.
  • Logos or map reproductions associated with your center/practice can also be incorporated into your personalized brochure order for center members. All other images are prohibited. Images can be attached in an e-mail using one of the following file formats: PDF, JPEG, or TIF. ***Files MUST be 300 dpi or higher.
  • There is a $65.00 charge for any revisions to EXISTING templates. All template fees waived on orders of 2500 or more brochures.
  • All revisions must be typed or clearly printed on a current brochure copy or new template form MUST be filled out with the revisions.

TIME FRAME...
  • Brochures require approximately 4 weeks for delivery from date of final approval.
*Only fully accredited center/laboratory members can include the AASM logo on personalized brochure orders. Provisionally accredited facilities may not include the AASM Logo.
